Program Overview
The goal of this continuing education article is to describe the importance of medication reconciliation in ensuring patient safety for the intended audience of pharmacists. After completing this program, participants will be able to describe the characteristics that medication reconciliation processes used in various health care settings should include. Participants will also be able explain methods of overcoming challenges related to medication reconciliation. Lastly, participants will be able to identify the role of the pharmacists in medication reconciliation (Pharmacist Only).
