  • Overview

    Medication errors occur in every practice setting of pharmacy whether hospital, community, specialty, mail order, or ambulatory care. Errors of medication use are even common once a patient has returned home. Medication errors occur for many reasons and every situation has a unique combination of contributing factors.  Almost all errors can be attributed to multiple factors and almost never will an error have a single contributing factor. This knowledge based program will focus on recognizing and preventing common medication errors and describing the roles of Root Cause Analysis and Failure Mode and Effects Analysis in identifying medication errors. This program meets and is approved by the Florida Board of Pharmacy as a Medication Error program and also meets Medication Error requirements of states other than Florida.
