Although errors are normal and are present in every profession, pharmacy has been the subject of intense scrutiny for the past decade, and the need to open up under scrutiny has challenged pharmacists to put their best feet forward. There are certainly many reasons for pharmacy error that exist well outside the individuals who practice pharmacy and the places where they practice. But laying the blame on third party programs that provide woefully inadequate reimbursement, prescribers who communicate unclearly, or patients who refuse to accept responsibility is beside the point. Pharmacists must themselves step up to the plate and offer solutions that are within their control.

As a profession, pharmacists speak from a regulatory perspective through their boards of pharmacy. In most states the regulatory agency responsible for pharmacists does more than licensing; it establishes standards for pharmacist practice and for the maintenance of a pharmacy practice site. It is through this self-governing regulatory agency that pharmacists can best show the public that patient care is the most critical concern to the profession, and that the standard is pharmacy is not just to protect public health but to promote public health.

In this program, the responses of two states to the problem of pharmacy error are highlighted. The two states are Florida and Washington. But these two case studies could be stories from any state, because there is no reason to think that pharmacy differs greatly in any of the 50 states. What is important about the experiences in these states is not that they are perfect. They do not necessarily serve as a model for all other states to follow. But they do provide useful information about pharmacy error, and they also describe how pharmacists can collectively learn from errors of the past and improve practices in the future.
