      More than 10 million patients visit their physician every year for heart failure. More patients require hospitalization for heart failure than all types of cancer combined. Heart failure usually gets worse with time. Pharmacist involvement in the management of chronic heart failure can improve quality of life and minimize hospitalization. This program will give pharmacists an understanding of the treatment goals for chronic heart failure, current and emerging treatment options and strategies to maximize patient quality of life, reduce economic burden and minimize follow on hospitalizations.
