      Patients with advanced illnesses frequently are taking 5, 10 or even more medications, often without a clear indication or purpose. Similarly, they are taking preventive medications, or using medications for symptom management that is no longer appropriate. Practitioners struggle with how to make these determinations, and discontinue inappropriate medications.
