    • Overview

      In this program, the participant will learn how to education their patients with heart failure to minimize hospitalizations which is the number one driver of heart failure related costs. Essential daily self-care skills will be reviewed including symptom assessment, dietary sodium assessment and daily weighing. Targeted patient resources will also be discussed.
