Pharmacy technicians play a vital role in supporting Medication Therapy Management (MTM), a service designed to optimize patient care, reduce hospital readmissions, and lower healthcare costs. This module introduces Medication Therapy Management (MTM) by exploring the Medicare Modernization Act of 2003 and its significance for MTM programs. It covers the evolution MTM initiatives aimed at reducing hospital readmissions and healthcare costs, outlines CMS requirements for MTM services under Medicare Part D, and identifies target population particularly those with multiple chronic conditions or on multiple medications. The module provides a concise overview of MTM’s role in enhancing medication management and improving health outcomes.
Module 2 will explore the CMS requirements for Medicare Part D programs to offer MTM services. These services will be defined to provide the learner with an understanding of the components of each type of MTM services. CMS also provides guidance on how MTM services may be delivered and to whom. Since many patients may reside in a Long-Term Care Facility, or have an authorized caregiver in their home, it is important to identify who may be present during a medication review or who may complete the review on behalf of the patient. Furthermore, a brief introduction to the Core Elements of MTM service will be provided to allow the learner to explore the steps of the MTM service and learn which components of the MTM service may be completed by a Pharmacy Technician. The Core Elements will be broken down further in Module 3.
In Module 3 the learner will further explore the Core Elements of MTM Service as each element is broken down and discussed in detail. Additionally, the documentation requirements for each of the Core Elements will be discussed. The CMS MTM Program Standardized Format for documentation provides guidance and templates which are required to be used for any MTM Service. The importance of using patient friendly terminology will be discussed, followed by examples and practice scenarios guided by the AHRQ Health Literacy Tool for Providers of MTM.
This module covers the process of identifying and scheduling patients for Medication Therapy Management (MTM) services. The module aims to equip learners with the knowledge to effectively identify eligible patients, prepare for MTM services, and create an appropriate environment for these appointments.
The fifth module in this course will discuss the patient interview and medication history. First, key terms such as medication adherence, medication intolerance, medication allergy, social history, and medical condition will be defined. Next, the Pharmacist and Pharmacy Technician roles in a patient interview will be compared to identify the differences in limitations during the medication history process. An emphasis will be placed on ensuring that the pharmacy technician is not counseling the patient during the review. Finally, the importance of active listening will be discussed because, aside from documentation, this is where the pharmacy technician’s role can be very impactful in the medication review. They can combine active listening with their knowledge of common drugs to identify potential low health literacy, cognitive impairment, cultural and social differences, inappropriate medication use, prescribing errors, or improper medication use technique.
This module focuses on developing the Personal Medication List (PML) and updating patient profiles during Comprehensive Medication Reviews. It emphasizes the importance of recording medication allergies, intolerances, and current medications, as well as documenting essential details on the PML, including strength, dose, form, route, frequency, duration, and administration instructions. The module also covers the documentation of all prescription and non-prescription therapies and highlights the use of refill history to calculate days’ supply and assess adherence. Overall, it underscores the pharmacy technician’s role in ensuring accurate medication documentation for effective Medication Therapy Management.
Although the pharmacist must conduct education and counseling to develop the patient’s Medication Action Plan, the pharmacy technician plays a vital role in helping to support the pharmacist’s efforts. This module will define common medication related problems and prescribing errors. The impact of such errors will be discussed, as well as what information the pharmacist may review to identify interventions and recommendations about the patient’s drug therapy. The pharmacy technician, once again, plays vital role in influencing how the pharmacist may tailor their education, counseling and questioning to the patient. During the patient interview, the pharmacy technician may ask questions to determine if the patient may be experiencing any side effects or if they are using their medication appropriately. The legal limitations for pharmacy technicians will be highlighted as applicable here.
The final module in this course will begin by briefly discussing how the pharmacist creates the Medication Action Plan (MAP) by compiling information and data gathered throughout the review process and the creation of intervention claims. Next, a discussion on claim submission for Comprehensive Medication Reviews and Targeted Medication Reviews will discuss the information required for each type of claim submission. The pharmacy technician can, again, play a vital role in communicating the pharmacist’s recommendations to the prescriber, documenting the prescriber’s response, and submitting claims. The importance of having a tracking system throughout the MTM service process will be highlighted. Finally, the learner will explore potential metrics that can be used to measure the success of a pharmacy’s MTM program.
